Unexplained Infertility: When Tests Are Normal But Pregnancy Hasn't Happened
15–30% of couples have unexplained infertility. Learn what "unexplained" really means, hidden factors doctors suspect, and why IVF often offers the highest success rates.
What is unexplained infertility?
Unexplained infertility is diagnosed when all standard fertility investigations appear normal, yet pregnancy does not occur after at least 12 months of regular unprotected intercourse. It accounts for approximately 15–30% of infertility cases. Couples typically have normal ovulation, normal semen analysis, open fallopian tubes, and a normal uterine cavity.
Possible hidden causes
- Subtle egg quality issues
- Sperm functional defects not seen on basic semen analysis
- Implantation abnormalities
- Subtle hormonal disturbances
- Genetic factors
"Unexplained" does not mean there is no cause — current testing may simply not identify the underlying issue.
Treatment ladder
- Timed intercourse — initial management (₹3,000–10,000)
- IUI — first-line fertility treatment; 10–20% per cycle (₹10,000–25,000)
- IVF — higher success after failed IUIs; 45–65% (₹1.5L–3L)
- ICSI — considered in repeated IVF failures; 40–60% (₹1.8L–3.5L)
Prognosis
Many couples eventually conceive naturally or with fertility treatment. IVF often provides the highest success rates when simpler treatments have not worked. A structured treatment plan with defined timelines reduces the emotional toll of uncertainty.
